A Letter From The Administrator
Dear Resident and Family members,
As the Administrator, I would like to Welcome you to Farmington Care Center. At our home, caring for the whole person is the philosophy that is reflected in the attitude and performance of every member of our team.
Additionally, we strive to exceed your expectations. In doing so, we understand that we must partner and participate with you in your plan of care. Communication along the way will be extremely important. This will include a brief meeting only a few days after you are admitted, interdisciplinary "Care Conferences", Discharge Planning Meetings and consistent communications with your caregivers.
We offer a variety of programs specially designed to meet the individual needs and goals of each resident. Our Rehabilitation Program is designed to meet the needs of those who have experienced a physical or cognitive loss of function. Our program utilizes a team concept that involves residents, families and professional staff who work together to restore maximum independence and resume a satisfying productive life.
Our Dementia Program offers a secured environment in which residents can utilize specialized services under the direction of a Full Time Program Director. In addition, Farmington Care provides post-acute medical and surgical services, intravenous therapy, wound care, hospice services and pain management.
Furthermore, Farmington Care Center recently achieved a Deficiency Free State Survey from the State of CT, The American Health Care Associations' Quality First Award and a "Five Star Rating" from the Centers for Medicare and Medicaid services.
